PREP Summer Capstone 2009

Application Requirements

• Completed application (on-line or paper application)
• Personal Statement (uploaded with on-line application or e-mailed separately)
• Recent headshot photo (uploaded with on-line application or e-mailed separately)
• Official copy of Transcripts mailed to the PREP office
• One completed evaluation form or recommendation letter (submitted on-line, through the mail or e-mailed separately)

All materials submitted within your application packet will be kept confidential and will be available only to the PREP Summer Capstone Admissions Committee. Accepted and alternate applicants will be notified in early April.
